There is a growing epidemic of chronic disease in the UK due to tobacco use, unhealthy diets, sedentary lifestyle and other risks. Whilst it is important to prevent these diseases from happening, it is also important to prevent them from getting worse.

Examples of chronic disease which require complex coordinated care include diabetes, asthma, COPD, heart failure and rheumatoid arthritis.  


It is very important to prevent the complications of a chronic disease once a person has a disease. Let us take diabetes as an example, if a person with diabetes goes without the care they need, their diabetes will probably get worse over several years. This can lead to complications such as kidney failure, blindness, heart attacks, strokes and so on.
